Why quarterly and not annual
Memory covers about two months plus one incident that stuck. An annual review is a recall exercise.
Both sides write, at the same moment, three or four lines each.
Written during the conversation, not afterwards from memory.
This sheet never determines pay. Run that separately or the answers here go strategic.
Nobody should be surprised at an annual review. If they are, something was noticed and not said, and that is a failure of the process rather than a revelation about the person. Put all four dates in the calendar now; the ones scheduled quarter by quarter are the ones that slip.
Each quarter is stored separately in this browser, so the sheet builds up over the year. Print all four quarters assembles them into one document for the annual conversation, which is the whole point of doing it quarterly.
